This service helps you easily register with your GP.

These forms can be emailed to angelhill.receptionadmin@nhs.net or you can bring the completed form into the Surgery. If you have an NHS number and know it, please complete this section on the form, this will assist us in finding your medical record.
You do not need proof of ID to register with a GP, but it might help of you have one or more of the following:
- passport
- birth certificate
- HC2 certificate
- rough sleepers’ identity badge
- hostel or accommodation registration or mail forwarding letter
If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
We may invite you to make an appointment with a doctor for a “New Patient Medical”
If you are staying in the Bury St Edmunds area and require medical assistance that cannot wait until you return to your usual Doctor you may register as a temporary patient.
For temporary registrations we will require your home address, your registered doctors details and where you are staying in Bury St. Edmunds.
